CHERRY HILL, NJ – Today, Congressman Donald Norcross (NJ-01) announced that the Camden Coalition of Healthcare Providers was awarded an $824,110 Accountable Health Communities grant from the Centers for Medicare & Medicaid Services (CMS).

“I am pleased to announce this funding that will help us address some key healthcare needs that are often overlooked for vulnerable community members,” said Rep. Norcross. “The Camden Coalition of Healthcare Providers was selected because it’s already spearheading productive collaborative efforts and this grant will allow the Coalition to continue leading the charge in health services partnership.”

"We are excited to move forward the work that we've been doing for the past 15 years in Camden,” said Dr. Jeffrey Brenner, Executive Director of the Camden Coalition. “Our work would not be possible without the partnerships we've built across the city, and this grant will allow us to strengthen those relationships as we work towards care that meets the needs of Camden's most vulnerable residents."

The Accountable Health Communities program supports local communities in addressing the health-related social needs of Medicare and Medicaid beneficiaries, such as housing instability, food insecurity, utility needs, interpersonal violence and transportation. The program is structured to addresses these needs by bridging the gap between clinical and community service providers.

The Accountable Health Communities program is authorized under Section 1115A of the Social Security Act and will begin on May 1, 2017 with a five-year performance period. Click here to view a list of all selected organizations and learn about how the program is structured.
